How do I join?
You can enroll online, mail-in the enrollment form, or call (808) 523-3111. After enrolling, call 523-3111 to make an appointment.
Please download the Oahu & Big Island enrollment form, or the Maui and Kauai form. The GE tax is county specific.
When does my membership start?
Savings commence once your payment is received and is good for one year from the date.
Will I have to wait before I am eligible to save on specific dental procedures?
Unlike traditional dental insurance there is no waiting period imposed for any of the treatments included in your plan, even the more complex procedures (such as root canals and crowns).
Do I have to file claim forms or request pre-certification?
There are no claim forms or pre-certification with Ho’ala, since the plan isn’t insurance. All unnecessary delays to your dental care have been eliminated!
I have dental insurance. Can I still join?
Yes. If you have both, you’ll be asked at your appointment which dental coverage you would like to use that day. Typically, people with dental insurance will enroll to receive coverage for benefits not covered in their insurance plan, or if their insurance plan has already reached its maximum.
I want to sign up others at my work. Do you have group plans?
Ho’ala business plans are offered for any business with two or more people. Please call (808) 523-3111 or email [email protected] to sign-up for a business plan.

There are three simple membership plans to choose from. Each 12 month plan begins when you enroll online.
